Who I Am & What I Do


Together Pathways is an independent NDIS Support Coordination practice founded and led by me, Effie Stagnitti.

My sole focus is supporting participants to build the right supports around them, understand their NDIS plans clearly, and use their funding with confidence and purpose.

I work with participants across the full NDIS spectrum, including those navigating early plan implementation, psychosocial disability, complex neurological conditions, acquired brain injury, behavioral presentations, and SIL or SDA pathways.

Whether your situation is straightforward or highly complex, my role is the same:
to bring clarity, structure, and strong advocacy to your NDIS journey.

Support Coordination is not just about linking services, it’s about ensuring supports are implemented properly, budgets are monitored, goals are actively worked towards, and your voice remains central in every decision.

At Together Pathways, your plan works for you, not the other way around.

ABOUT ME 


Hi, I’m Effie.

I’m a Melbourne-based NDIS Support Coordinator and the Founder of Together Pathways.

My background combines professional experience in the disability sector with lived insight supporting close family members with intellectual disability and acquired brain injury. Navigating complex systems personally has shaped how I practise professionally — with empathy, structure, and strong accountability.

I understand that behind every NDIS plan is a family trying to make things work. I understand how overwhelming funding, reports, reviews, providers, and systems can become. And I understand how important it is to have someone steady and strategic in your corner.

My approach is calm, organised and solutions-focused. I work closely with families, therapists and providers to ensure supports are coordinated properly, funding is maximised appropriately, and nothing falls through the cracks.

Outside of work, I’m a wife, a mum, and someone who values family deeply. I believe in hard work, integrity, and advocating strongly when it matters most.

My philosophy is simple:

Every participant deserves clarity, respect, and genuine choice and control.
